Why does it occur?

What exactly causes a pilonidal sinus or abscess is not known specifically, owing to the various reasons that aid to triggering it. But then, it is also believed to be a combination of hormonal changes during puberty, hair growth, and friction that clothes can cause from spending a long time in a seated position.

Sometimes while sitting, there can be a friction that forces the hair growing in the cleft of the buttocks area cause the hair to settle under the skin and grow inwards. The body considers this ingrown hair as foreign with an immune response against it which in turn forms an abscess or a cyst around that hair. In some cases there can also be multiple sinuses that grow one after the other in a cluster forming a bigger cyst.
